Informed Consent to Telehealth

1. Nature of Telehealth

Telehealth is the delivery of health care using electronic communications rather than an in-person visit. Your visit may be synchronous (live video or phone) or asynchronous(store-and-forward review of a written intake, photographs, and lab results), depending on your state's law and the clinician's judgment. Your clinician may determine that telehealth is not appropriate for your condition and require an in-person evaluation.

2. Expected Benefits

• Access to licensed clinicians without travel or waiting rooms.
• More convenient scheduling and faster follow-up.
• Discreet care for sensitive conditions.

3. Possible Risks

• Information transmitted may be insufficient for sound medical decision-making, requiring additional tests or an in-person visit.
• Delays in evaluation or treatment could occur due to technology failures.
• Rarely, a security breach could allow unauthorized access to your information.
• Absence of a physical examination may result in a missed or delayed diagnosis or an adverse drug interaction.
• Photographs or images transmitted may be of insufficient quality for clinical assessment.

4. Alternatives

You may choose an in-person visit with a local provider, urgent care, or another telehealth provider instead. Declining telehealth will not affect your right to future care.

5. Medication and Follow-Up

If a prescription is issued, it will be sent to a licensed pharmacy for dispensing. You agree to follow dosing instructions, complete any required lab work or check-ins, and report side effects promptly. Your clinician may adjust or discontinue treatment at any time. Prescriptions are not guaranteed, and controlled substances are not prescribed through the Services.

6. Records and Privacy

A record of your telehealth visit is maintained in your medical record and protected under HIPAA and applicable state law, as described in our HIPAA Notice of Privacy Practices. Visits are not recorded on video without your separate written consent. You may request a copy of your record at any time.

7. Your Rights

• You may withhold or withdraw this consent at any time without affecting your right to future care.
• You may request an in-person referral at any point.
• You have the right to know the name, license type, and licensing state of your treating clinician.
• You may ask questions about your treatment before agreeing to it.

8. Emergencies

Telehealth is not appropriate for emergencies. Call 911 or go to the nearest emergency department for urgent symptoms, and call or text 988 for a mental health crisis.

9. Financial Responsibility

You are responsible for the fees disclosed at checkout. DirectRxCare does not bill insurance, and telehealth visits and compounded medications may not be reimbursable by your plan.

10. Acknowledgment

By completing a medical intake and submitting payment, you acknowledge that you have read and understood this consent, that you have had the opportunity to ask questions, and that you voluntarily consent to receive care via telehealth from a DirectRxCare-affiliated clinician licensed in your state.
